1.Information about the product.

2.Cost price of product based upon national quantity purchase.

3.Company principal or representative contact name, address, phone, fax.

4.Product sample or photo. Samples submitted cannot be returned unless arrangements are made prior to submitting actual sample. Due to the number of products submitted, Genius Babies! cannot pay for samples.
